Insurance: 

 

All commercial insurances will be billed for you. 

Co-pays are due at the time of your visit.

If we are not in your insurance company's preferred network, your insurance benefits may not pay us as they would an in-network provider, and you will be responsible for the difference.

If you are a self-pay patient, payment is due at the time of service.

Additional Fees: 

 

There is an additional $25 fee for completion of forms for use outside of the medical chart including, but not limited to following:
